Program Requirements and Evaluation
The University of Miami at FAU selects one fellow each year to participate in the Palliative Medicine Fellowship Program.
Prerequisites
• Successful completion of an ACGME accredited residency program
• Board certification or board eligibility in Internal Medicine or Family
• Medicine (Consideration will be given to other board-certified or
• board-eligible disciplines, depending on the candidate’s
• professional goals)
• An unrestricted license to practice medicine in the state of Florida at the
• time training begins
• A minimum one-year commitment
Prior to beginning training, the selected fellow must obtain medical staff privileges at Hospice of Palm Beach County, JFK Medical Center and other affiliated sites, as required.
Application Packet
Your complete application packet must include:
• A completed application form
• Curriculum Vitae
• A one-page statement explaining your interest in palliative medicine
• A copy of your current state medical license and DEA certificate
• A notarized copy of your Flex, USMLE and/or National Board Certificate
• A notarized copy of your medical diploma
• A copy of your Social Security card
• A recent photo attached to the application form
• Three letters of reference that attest to your personal qualifications,
• scholastic achievements and clinical capabilities
• A notarized copy of your ECFMG Certificate (Foreign graduates only)
• A copy of your green card, or proof of U.S. citizenship or Visa status.
Selection Process
• A quorum of the HPBC Education Committee will interview the most
• competitive candidates
• Candidates are evaluated based on prior training, demonstrated clinical
• competence, academic achievements and personal commitment to
• end-of-life care
Interviews are conducted by appointment. The committee will contact you to schedule an appointment if you are selected for an interview.
